Positive vs negative canthal tilt: what it means and how to measure yours
Last updated: September 2026
Canthal tilt is the angle of the line from the inner corner of your eye (the medial canthus, by the nose) to the outer corner (the lateral canthus, by the temple), measured against a level line through both eyes. Outer corner higher than inner: positive. Level: neutral. Outer corner lower: negative. It became a search term because TikTok turned it into a verdict. It is a measurement, and you can take yours in seconds with the free canthal tilt test.
The ranges, in degrees
| Tilt | Called | Reads as | How common |
|---|---|---|---|
| below 0 | negative | tired, sad, older | a minority; more common with age as the outer corner descends |
| 0 to 2 | neutral | calm, open | common |
| 2 to 6 | positive | alert, awake, the almond shape | the most common range, roughly half of faces |
| above 6 | strongly positive | cat eye | rare |
Two eyes rarely match. A one to two degree difference between them is normal and usually a mix of real asymmetry and a slightly turned head. A difference above three degrees on a straight photo is worth checking against the symmetry test to see whether the rest of the face follows.
Why the tilt reads the way it does
A positive tilt reads as youthful because the outer corner descends with age: the tendon that anchors it loosens and the cheek beneath it drops. It reads as alert because the eye narrows toward the temple, which is the shape eyes take when someone focuses. A negative tilt reads as tired for the opposite reasons, and it is the tilt most people search for because they have noticed it in photos.
The key word is photos. Tilt is the eye measurement most distorted by the camera. A camera held above eye level makes any tilt read more positive; a camera below eye level makes it read more negative. A smile lifts the cheeks and the outer corners; a squint drops them. Overhead light shadows the outer corner and the landmark shifts. Most negative tilt people worry about is a camera held at chest height.
How to measure it properly
- Camera at eye level, arm's length or further, even light from the front, neutral face, eyes open and relaxed.
- Face the camera straight on. A turned head compresses one eye.
- Run the photo through the canthal tilt test. It finds the four corners with a landmark model in your browser, levels the head so a tilted phone does not count as a tilted eye, corrects for camera height, and reports each eye in degrees.
- Retake if it flags the photo as turned, tilted or off eye level. Compare only photos taken the same way.
To do it by hand: print the photo or open it in an editor, draw a line through both pupils, then a line from the inner corner to the outer corner of one eye, and measure the angle between them with a protractor. The test does the same thing with the lines placed the same way every time.
What changes canthal tilt, and what only looks like it does
- Surgery. A canthoplasty cuts and reattaches the outer corner's tendon higher; a canthopexy tightens it without cutting. These are the only things that move the anchor point. They are real procedures with real risks.
- Camera height. The largest effect in photos and the cheapest to fix. Eye level, every time.
- Brow position. A brow tail groomed a little higher makes a neutral tilt read positive; a low brow tail makes it read negative.
- Puffiness. Fluid under the eye and around the outer corner rounds the eye and drops the corner for a day. Sleep, salt and alcohol.
- Age. The outer corner descends slowly through the forties and fifties. This is the one direction that is not a photo artefact.
- Not mewing, not eye exercises, not tape. No measured effect on the tendon or the bone.
Canthal tilt and attractiveness
Positive tilt is one of the traits raters weigh in the eye area, and the eye area is the region they weigh most. It is not a requirement. Faces rated attractive sit across the neutral and positive ranges, and a slight negative tilt on an otherwise harmonious face reads as soft rather than tired. If you want the number in context, the hunter eyes test reads tilt together with eye openness and brow position, and the attractiveness test reads it with the rest of the face.

FAQ
What is a positive canthal tilt?
The outer corner of the eye sits higher than the inner corner. Measured against a level line through both eyes, anything above about 2 degrees reads clearly positive.
Is negative canthal tilt bad?
It reads as tired in photos, which is why people ask, but it is common and it is one line of the eye area. Check the camera height first: a camera below eye level makes any tilt read more negative.
How do I measure canthal tilt?
Draw a level line through the pupils and a line from the inner corner to the outer corner of one eye, and measure the angle between them. The free canthal tilt test does it from a photo in your browser.
What is the ideal canthal tilt?
There is no ideal number. Roughly half of faces measure between 2 and 6 degrees positive, and faces rated attractive sit across the neutral and positive ranges.
Can canthal tilt be fixed without surgery?
The anchor point cannot move without surgery. How it reads can: eye level camera, brow grooming and less puffiness change the photo more than most people expect.
